Confidentiality and Privacy During Sessions

Clients have the right to a private and secure environment during their sessions. No individuals will be present during a session unless the client has given explicit consent for them to be there.

Sharp Minds Therapy and its affiliated healthcare professionals may collect relevant personal and medical information, which will be securely documented in the client’s digital file within our internal system.

All personal and medical information shared with Sharp Minds Therapy is strictly confidential and will only be disclosed to third parties with the client’s written consent.

If a client wishes to authorize the release of their information to a third party, they must complete and sign a Release of Information form.

In accordance with our privacy policy, audio or video recordings of counseling sessions are strictly prohibited by both clients and Sharp Minds Therapy staff.


Client Privacy Policy
At Sharp Minds Therapy, we are committed to protecting the privacy and confidentiality of our clients. Our policies fully comply with Ontario’s Personal Health Information Protection Act (PHIPA), which governs how personal health information is collected, used, and disclosed.
Client Rights Regarding Personal Information
Clients have the right to withhold personal information from Sharp Minds Therapy.
Clients also have the right to restrict the sharing of their information with third parties—except where disclosure is legally required, such as situations involving risk of harm to the client or others.
Access to Personal Records
Clients may request access to the personal information we have on file about them.
All requests must be submitted in writing and will be processed in accordance with applicable privacy regulations.

As a Registered Social Worker, I hold all client information in strict confidence and comply with any applicable privacy and other legislation. I shall disclose such information only with client consent or when required or allowed by law.
